Senior Care Options: one plan for Medicare and MassHealth
Senior Care Options is a free health plan for people 65 and older with MassHealth Standard that combines Medicare and MassHealth benefits with a care coordinator. In 2026, two SCO plans serve all of Barnstable County: Fallon Health NaviCare and Tufts Health Plan Senior Care Options.

Who qualifies
You can join if you:

- Have Medicare Parts A and B and qualify for Part D
- Live at home or in a long-term-care facility in the plan's area
What it offers
One plan and one card cover your Medicare and MassHealth benefits. A care team, including a care coordinator, helps arrange doctors, home care and social support. Some plans cover extra dental work. SCO is voluntary — you can stay in regular MassHealth and Medicare.
